John K. Cuddeback M.D., Ph.D. can be located at the Department of Health Policy & Epidemiology, Shands Hospital, University of Florida, P.O. Box 100177, Gainesvillle, Florida 32610. Phone (954) 395-8035; Fax (954) 395-8047. Dr. Cuddeback received his Ph.D. in Physiology (I977) and his MD (I979) at the University of Indiana. He was Vice President of the University of Florida Shands Health Policy Department and serves in a part time faculty appointment at the UF Department of Health Policy and Epidemiology.
